HOW DO I....

OBTAIN A REPEAT PRESCRIPTION?
Repeat prescriptions are issued by the doctor and are normally for patients on long term treatment. The computerised slips should be left in the box in the porch, posted to the surgery or faxed (Fax no 020 8309 7020). The prescriptions can be collected only after two working days. We will post them if an SAE is provided. If you require a medicine that is not listed on a repeat form, please put it in writing the reason for the request and it will be assessed by the doctor.

HOW DO I....

OBTAIN A SICKNESS CERTIFICATE?

A self-certificate (SC2) covers illnesses of seven days or less and can be requested from the receptionists. If illness lasts longer than seven days a doctor’s certificate may be necessary.
